The forms on this page are divided into five broad categories. Not all students will need to complete all forms. If you are ever unsure of whether or not you are completing the correct form, please contact our Office by e-mail or by calling either 847-735-5010 or -5015.
All forms on this page are for the 2008-2009 academic year.

(3) Your Financial Aid Announcement.
The Financial Aid Announcement Guide provides important details about the scholarships, grants, loans and work-study which you have been offered. You will find a breakdown of the costs, scholarship renewal requirements, definitions of grants, descriptions of loans, information about the amount you will need to pay, and choices you have in making payment. (4) Loan for Students and Parents.
- Students who have been offered a Perkins Loan or Stafford Loan for the first time must complete two items for each loan: (1) an entrance interview; (2) a Master Promissory Note. See Student Loan Process
- Students who have been offered or who need to use an "Alternative Loan" must complete a promissory note. See Ways to Pay (option #4)
- Parents who plan to use the PLUS Loan must complete two steps: (1) loan pre-approval; (2) a Master Promissory Note. See Ways to Pay (option #3).
