Off-Campus Study Application Instructions
STAGE 1 INSTRUCTIONS
1. Read about off-campus study on the College web page and then make an appointment with Jan Miller, Assistant Dean of Faculty for Off-Campus Programs (Johnson B-177) to discuss possibilities and to obtain the Stage 1 petition. ALL applicants, including for In the Loop, must complete this process through the Off-Campus Programs Office.
2. Download the following forms:
a. Your current unofficial transcript
b. Advisor Recommendation form
c. Dean of Students Recommendation form (page 1)
e. Behavior Policy/Harassment Statement form
3. Talk to your advisor about your plans. Sign your name on the indicated line of the Advisor form and give it to your advisor who will return it directly to the Off-Campus Programs office.
4. Sign your name on the indicated line of the Dean Recommendation form and take it to the Office of Student Affairs (738 Rosemary Road. No appointment is necessary). You do not need an appointment but must submit the form in person. The Dean will complete this form and return it directly to the Off-Campus Programs office.
5. Take the Financial Aid Request form to the Financial Aid Office (Patterson Lodge). If the Director of Financial Aid (Jerry Cebrzynski or his designee) is not available, leave the form at this office and pick it up in person later. You must sign this form AFTER the Director of Financial Aid has signed it so that you know the eligibility status of your aid package. Return the form to the Off-Campus Programs office. You must complete this form even if you receive no financial aid.
6. Sign the Behavior Policy/Harassment Statement form.
7. Submit the completed Stage 1 Petition, your unofficial transcript, the Financial Aid Request form, and the Behavior Policy/Harassment statement to the Off-Campus Programs office. The deadline to submit completed Stage 1 documents—for either fall OR spring participation in the following academic year— is November 15.
8. When ALL the required items—including the Advisor and Dean forms—have been received in the Off-Campus Programs office, you will be notified of your eligibility status.

STAGE 2 INSTRUCTIONS
When you have received confirmation of eligibility to proceed with Stage 2, complete the following steps:
1. Obtain the Stage 2 petition from the Off-Campus Programs office.
Note: In the Loop applicants will complete this process through the Center for Chicago Programs.
2. Select your program and apply as indicated below.
3. Download an updated unofficial transcript.
4. Complete the program application for your selected program and submit it as follows:
For LAKE FOREST COLLEGE PROGRAMS:
1. Download the application form for your chosen Lake Forest program. Application forms may be found by clicking on Lake Forest Programs, then on the specific program link, and finally, on that program page, by clicking on the application link.
2. Complete and return the application to the Off-Campus Programs office.
3. Download two faculty recommendation forms from the College web site.
4. Ask two classroom teachers to complete a recommendation form and return it to the Off-Campus Programs office. For Granada and Paris programs, one recommendation should come from a language teacher. You may use this same form for an optional third recommendation from a non-teaching staff person.
The campus program committee will notify you of your acceptance.
For ACM PROGRAMS:
1. Complete the on-line application form on the ACM web site.
2. Print a copy of your application responses and submit it with the signature page (only your own signature is required) to the Off-Campus Programs office.
3. Ask your teacher recommenders to complete the ACM recommendation form (on the ACM web page under Apply Now) and return it directly to the ACM Chicago office.
4. An ACM representative will notify you of your acceptance.
For WASHINGTON SEMESTER PROGRAMS:
1. Complete the on-line application form, print a copy and submit it to the Off-Campus Programs office.
2. Print 2 faculty recommendation forms, give them to two classroom teachers and ask them to return their completed forms to the Off-Campus Programs office. (These recommendations are in addition to any requested by the Washington Semester Program).
3. A Washington Semester representative will inform you of your acceptance.
For OTHER PROGRAMS:
1. Complete the application form—on-line or hard copy found in a program catalog. The Assistant Dean of Faculty for Off-Campus Programs can complete forms requiring the signature of a campus official.
2. The sponsoring program will notify you of your acceptance status. Send a copy of that acceptance letter to the Off-Campus Programs office.
BE SURE TO KEEP A COPY OF YOUR COMPLETED APPLICATION FORMS.

COURSE SELECTIONS:
When you have been accepted by your selected program and have decided to participate, download the Course Selection Form. Obtain the required signatures and return it to the Off-Campus Programs office.
Participants in Lake Forest, ACM, and Washington Semester programs need to submit this form only if they wish credit toward a major or minor.
Participants in all other programs must complete and submit the Course Selection form.

SUMMER STUDY
Students wishing to earn transfer credit for summer study abroad must complete, and submit—prior to participation—the necessary forms to the Off-Campus Programs office.
Requests to earn credit through direct enrollment in a foreign university (including in Canada) must be submitted to the Academic Appeals Board for approval. The amount of credit awarded for summer programs will depend on the number of semester hours designated for each course. Check with the Registrar for exact credit information.
The summer petition form, for both direct enrollment or participation in an approved program, must be obtained from the Off-Campus Programs office, Johnson B-177.
For information regarding summer study in the United States, please contact the Registrar’s office.
There is no Lake Forest College financial aid for summer study.
