In compliance with Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act, Lake Forest College recognizes that qualified students who have diagnosed disabilities, including learning, physical, and psychological disabilities, are entitled to an equal opportunity to benefit from the educational programs of the College and that reasonable academic accommodations may be necessary to provide that opportunity.
Students with disabilities are encouraged to use the academic support services available to all Lake Forest College students. Services for students with learning, physical, and medical disabilities are coordinated through the Learning and Teaching Center, while services for students with psychological disabilities are coordinated through Counseling Services. Students requesting reasonable accommodations in the classroom must provide the appropriate documentation as outlined below.
Learning Disabilities/ADD/ADHD
Students with learning disabilities/ADD/ADHD must request accommodations by submitting their most current (within three years) diagnostic testing reports. The testing documentation must be administered by a professional who is qualified to diagnose learning disabilities/ADD/ADHD. Testing reports should include cognitive, intellectual, and achievement testing scores and summaries, and should be submitted to the Learning Support Specialist of the Learning and Teaching Center (contact information below). At the beginning of each semester, the student must meet with the Learning Support Specialist to review documentation and accommodations. After the student (or guardian if the student is under 18 years old) gives written permission to notify his/her professors, letters which outline specific accommodations will be sent. All information pertaining to learning disabilities/ADD/ADHD is confidential and cannot be released without the consent of the student (or guardian, if the student is under 18 years old).
Physical/Medical Disabilities
Students who request reasonable accommodations due to physical/medical disabilities must submit documentation by a health care professional describing the nature of the condition and educational recommendations. Information should be submitted to the Learning Support Specialist of the Learning and Teaching Center. Before each semester, the student must schedule a meeting with the Learning Support Specialist to review his/her needs for the semester. All information pertaining to the disability is confidential and cannot be released without the consent of the student (or guardian, if the student is under 18 years old).

Psychological Disabilities
Students who request accommodations for psychological disabilities must provide written documentation from their current mental health professional as to diagnosis, treatment plan, and/or medication management to Counseling Services. The student must take the following steps in order to obtain accommodations.
It is the student's responsibility to meet with their professors and arrange for the accommodations. At the beginning of each semester, the student must schedule a meeting to review accommodations and request new letters.
